How long should lithium salts therapy be continued in the prevention of affective disorders?

Undesirable side effects of tong-term using lithium for preventive purposes as well as the risk connected with discontinuation of effective lithium therapy are discussed. Relapse after lithium discontinuation was noted in 75% of patients during a 2-year follow-up. Discontinuation of the treatment should be attempted only under circumstances exposing the patient to an increased risk of relapse.

Potentialization of antidepressive effect of drugs with lithium carbonate

Lithium was supplemented to antidepressant medication in 64 patients with major depression meeting the criteria for refractoriness to treatment. All the patients had previously undergone at least two treatments (pharmacological or ECT) with no satisfactory results. In the reported supplementary treatment with lithium carbonate the drug was introduced after at least 4 weeks of antidepressant medication. The dosage was targeted to produce serum lithium level between 0.6 and 0.8 mEq/l. After 4 weeks since lithium introduction symptoms of depression subsided in 35 (56%) of the treated patients. The remission was significantly more often noted in patients with depressive symptoms in the course of bipolar affective disorder (80%) than in those with unipolar affective disorder (48%): moreover, it was more frequent in patients with less pronounced depression (73%). In 24 cases a marked amelioration of the patients' psychiatric status occurred as early as within a week from the supplementary lithium therapy.

Immunological function in refractory depression

In 60 patients (11 males, 49 females aged 25-66 years) in the acute phase of major depression and prior to starting pharmacotherapy Creactive protein (CRP) and alpha-I-acidic glucoprotein (AGP) levels, as well as AGP microheterogeneity were checked. In the group under study 34 patients met the criteria for drug-resistance (no satisfactory results had been obtained in two previous successive pharmacological treatments). These patients, as compared to the other 26 subjects, were characterized by a longer duration of both the disease from the onset, and of the acute phase. A comparison of the two groups revealed that drug-resistant patients had higher AGP serum levels, higher values of the AGP microheterogeneity index, and a larger number of monocytes than had the remaining subjects. Changes found in patients with refractory depression may be interpreted as manifestations of immunological hyperactivation. This might suggest that signs of such hyperactivation are associated with reduced efficacy of medication in patients with major depression.

Effectiveness of cognitive therapy in the treatment of depression has been confirmed by numerous studies. There is ample evidence that best results – in terms of the remission period duration and quality – can be obtained by a combination of pharmacotherapy and psychotherapy. Due to the patients' depressive mode of thinking psychological methods in the treatment of depression are associated with particular difficulties. Examples of difficulties that may he encountered in the course of therapy are presented.

Evaluation of prevalence of untreated depressive disorders among the elderly

Depressive disorders prevalence was estimated in two populations of the elderly (over the age of 70): a) residents of nursing homes, and b) pair-matched controls living in their own homes. Persons with major physical or neurological disorders were excluded. Subjects in the study were persons never treated for depression in the past. The prevalence rate of depressive disorders was found to be high (up to 1/3 of the population). These disorders were over three times more frequent among residents of nursing homes. The frequency of occurrence of depression among the aged persons actively participating in the community life and keeping up more regular family contacts was lower.

The effect of extramural care type on the costs of treatment of schizophrenic patients

The unit of analysis were 179 five-year periods during which schizophrenic patients received one of two types of extramural care (delivered either by a traditional mental health facility, or by a Home Treatment Team). The provision of intensive community care reduced Home Treatment patients' length of inpatient stay by 59%, increased their community tenure by 57%, and in comparison to the traditional outpatient care group, reduced the direct costs of treatment by 42%.
